Signs of a human/door interface failure?

Not just a hospital thing, see these controls all over the place but this 'creative arrow addendum' highlights the issue:

• The door 'Touch to Exit' plate on the wall, but not that close to swing doors which have push-plates. Pushing those does nothing as it is a powered locked controlled door.
• The 'Door Release', also green, is a mystery?
• An 'Emergency Door Release', also green, alongside the normal one

I suspect any one of these three is only pressed, from the arrows two by mistake, after pushing the door push plate fails to open them...
